Our Care Pathway.

Clear, Structured, and Always Safe.

Starting complex care follows a clear process. Once the HSE approves support, our clinical team works directly with you to shape a care plan that fits daily life. Where hospital specialists are involved, we coordinate their input too — making sure the right people and the right plan are in place from the start.

Step 01.

HSE Approval.
Your local CHO or hospital team approves a package of care, setting the hours and level of support. Families may request Obsidian as their provider at this stage.
Step 02.

Clinical Assessment.
A senior nurse carries out a full assessment of your loved one’s health, routines, and family circumstances.
Step 03.

Personalised Care Plan.
Together with families, hospital specialists, and HSE community teams, we design a plan covering both interventions and daily living.
Step 04.

Safe Setup.
Before care begins, we put the right team, equipment, and medication protocols in place. Escalation procedures are agreed so everyone knows what to do in an emergency.
Step 05.

Care in Place.
Small, stable teams provide reliable daily support. Nurses deliver clinical interventions while carers assist under their supervision where required.
Step 06.

Ongoing Reviews.
Care plans are reviewed at least every three months, after any health event, or whenever a family requests it. Our Clinical Manager monitors progress continuously and adapt as needs evolve.
